It's actually a nickname. When he was coming through there was another Juninho as well (the one who played for Middlesborough). So Juninho (the freekick specialist) became Juninho Pernambucano (because he's from the state of Pernambuco) while the other Juninho was known as Juninho Paulista (he's from Sao Paulo) to differentiate between them.
