O'Neill has no fresh injury concerns but may decide to freshen things up following the recent run of poor results.
Wes Brown has returned to training following a lengthy lay-off with a knee problem dating back to the end of January, although the game is likely to come too soon for him.
Titus Bramble (Achilles) is likely to be included in the squad once again after making the bench at Fulham last week, but full-back Kieran Richardson and midfielder Sebastian Larsson (both hernia surgery) will miss out.
Manchester United's Chris Smalling has been ruled out of Euro 2012 with a groin problem, leaving Ferguson with limited options in defence for this game.
Jonny Evans also missed the 2-0 victory over Swansea last weekend so Phil Jones is a certain starter for the clash with the Black Cats.
Danny Welbeck is sidelined due to an ankle injury, meaning that Javier Hernandez is likely to start alongside Wayne Rooney in attack once again.
Anderson, Darren Fletcher and Nemanja Vidic remain long-term absentees.
