Yeah, before Philadelphia entered the league, the closest team to D.C. was New York, which was still 200 miles away.

No, not many people travel. But we do have some closer teams now. NY-Philly, Philly-DC, Portland-Seattle, are some examples of places where people will travel to, but it's never more than a couple hundred. And because of the distances, people will usually only travel to Saturday afternoon/night games. The one I went to in Columbus was Sunday night at 5pm, and there were only about 30 United supporters, who had made the 7 hour drive (it was just 5 hours for me and my girlfriend though).
What is more likely is that there are people in cities with MLS teams that are from other MLS cities, and will go when their team is in town. With the amount of moving in the country, its not uncommon to see 25-50 of those types at the games.

Yeah, there are people who travel cross country for games -- and they do so by flying and staying the weekend. I envy them!

I've been to a cup final (Ahly v Harras Hodoud) and a friendly match (AC Milan v Club America). I don't know if those count as away.
I've also been to an Zamalek v Ahly match, but both teams play their home games in the same stadium anyways.
