Yeah, that's true, he has tinkered A LOT...
Prior to the start of the season, everyone was buzzing about his comments regarding the almost mythical 4-2-4 formation, and that he was going to bring this formation to Juve. However, no-one has been able to completely execute such a formation to perfection quite like the Brazilians did during the time Pele was playing (especially WC 1970). It's certainly not a formation that is often used in modern day football. Conte's comments generated a lot of excitement, nonetheless.
He initially planned to go with the "4-2-4," which eventually evolved into a 4-1-4-1 after Juve's first few matches. As you already pointed out, against certain opponents (sometimes during the match) Conte changed the team's shape to a 4-2-3-1 (v. Milan), and 4-2-2-2/4-4-2. During the latter stages of the first half of the season Conte began implementing a 4-3-3 system. The team actually plays quite well in this shape.
Also, Conte tried the 'beating them at their own game' routine against Napoli and Udinese by playing a 3-5-2 formation. This worked against Napoli (to a certain extent), but somewhat failed against Udinese because everyone seemed a bit tired or out of sync.

Now, in regards to FIFA. If you want realism for your CM then try messing around different custom formations and tactics, tailoring them to how you think Juve should play. You seem to already know a great deal about the different shapes that Juve has played in this season.
This year's Juve has a ridiculous work-rate (no surprise there...Conte is a hardass), dominates possession and for the most part they keep the ball on the ground, only resorting to crossing when there are no other options or when Pirlo is attempting to deliver that "killer" ball. He has been our main man in terms of unlocking opposing defenses. So when you're trying to replicate these characteristics in a video game, think lots of shots, passes, possession, and a high tempo.
Personally, the only formations I use with Juve are the 4-1-4-1 (modified) and the 4-3-3. Sometimes I go with a tweaked version of a 4-4-2.
